Amber Christie, RN

Manager of Care Coordination, Community Care Alliance

Amber Christie, RN, is the Manager of Care Coordination for the Community Care Alliance. In this role, Amber is responsible for the growth and maintenance of the Care Coordination Programs. Her duties include recruiting and managing patients enrolled in care management services, ensuring the completion of annual wellness visits, following up on all elements of the preventive plan of care, and discussing advance care planning with patients.

Amber was born and raised in Montrose, Colorado, and attended the University of Wyoming, where she earned her Bachelor of Science in Nursing. She returned to the Western Slope and made her home in Delta.

Amber has worked in the healthcare industry for over 20 years. She began her career at Montrose Memorial Hospital as a CNA, Ward Clerk, and Telemetry Tech during holiday and summer breaks before graduating with her RN in 2003. She continued at Montrose Memorial Hospital as an RN on the Medical-Surgical Floor and, in 2004, transitioned to the ICU, where she worked until 2012. She then joined Volunteers of America Home Health of Western Colorado as a Case Manager and Wound Care Certified Nurse for the Delta County area before joining the Care Coordination Team.

Amber is passionate about working with rural patients and families, advocating for and helping them achieve their healthcare goals. Working in a rural healthcare setting often requires creative thinking and advocacy to achieve these goals.

Amber and her family live and work on their farm in Delta, where they raise a variety of crops and 4-H lambs. She supports her daughters as they travel for their many sports and show endeavors. Her love and passion for agriculture is evident in her continued work with youth involved in Montrose and Delta County 4-H programs, her role in leadership for the Delta County Bred and Fed program, and her position as Sheep Superintendent for the Delta County Fair.
