Preserving Rural Healthcare Through Transformation

The Community Care Alliance (CCA) was formed in 2015 by Western Healthcare Alliance to serve members as they embarked on the journey towards alternative payment models and population health.

Today, the CCA serves as a Clinically Integrated Network (CIN) for value-based contracting. Our CIN is a partnership of providers and hospitals collaborating to receive reimbursement based on high-quality, low-cost care.

Strategic Objectives

Support independent rural hospitals and practices in the transition to value-based care.

Leverage the network to improve the quality and efficiency of patient care and maintain cost control mechanisms across the state of Colorado.

Share accountability and rewards for achieving quality and performance benchmarks.

Optimize medical home, team-based care models, and population health management infrastructures.

Enhance existing relationships and partnerships with providers, community organizations, and public health.

Provide quality data reporting training and assistance, robust clinical informatics, and personalized operational coaching.

Collaborating together and taking healthcare delivery from volume to value, improving quality, greater patient outcomes, and lowering the cost of care.
