The Community Care Alliance’s (CCA), Clinically Integrated Network (CIN) brings together independent hospitals and primary care providers to deliver seamless, high-quality care. By working together, we ensure communities receive comprehensive, efficient, integrated primary care, tailored to their unique needs. With a focus on innovation and patient-centered care, we strive to enhance health outcomes and overall satisfaction.
Through our partnerships with ProviDRs Care and the ACO National Network, the CCA is launching a community driven health plan that allows rural employers to take control of their healthcare costs and outcomes through a self-funded ACO. In partnership with CCA’s CIN providers, communities and local employers will enjoy the benefits of reduced healthcare expenses, improved employee health, and enhanced healthcare experience.
Physician owned, ProviDRs Care Network provides the tools and resources for providers and integrated delivery systems to work directly with self-funded employers with as few as five employees and as large as 50,000 employees. Through the network, providers have access to data to help manage their populations and achieve greater shared savings through value-based payments tied to greater outcomes.
