Farrah Smilanich, RN

Regional Care Coordinator, Community Care Alliance

Farrah Smilanich, RN, is a Regional Care Coordinator for the Community Care Alliance. In this position, Farrah is responsible for the growth and maintenance of the Care Coordination Programs for selected facilities.

Farrah has been a registered nurse for over 17 years. She earned her nursing degree from Mesa State College and later graduated from Colorado Northwestern Community College. She began her nursing career in rural Colorado as a medical-surgical and labor and delivery nurse. In 2013, she transitioned to pediatrics, working as a nurse for the only pediatrician in her rural community of Craig, Colorado. In 2015, Farrah took on the role of Public Health Nurse Manager for Moffat, Routt, and Jackson counties. Most recently, she worked as the Integrated Practice Manager at a Federally Qualified Health Center in Craig before joining the Care Coordination Team. She is proud to continue advocating for rural healthcare.

A Colorado native, Farrah was raised in Grand Junction, where she met her husband of 23 years. They have four children who keep them very busy. Farrah coaches a local youth volleyball team and enjoys watching her kids participate in baseball, basketball, volleyball, and track. In her free time, she and her family enjoy spending time outdoors hunting, fishing, camping, and hiking.
