San Luis Valley Health’s Care Coordination team was recognized for their partnership with Center for Restorative Programs (CRP). Carey Deacon, Director of CRP, awarded the Care Coordination team at SLV Health with an award. “SLV Health has been really supportive, and we wouldn’t be able to do all that we do without the team here.”
